As part of our commitment to safety and providing regulated medical treatments, we are required to verify the identity of every patient. Here is some guidance on the types of ID we accept.
Accepted Forms of Photo ID
To be accepted, your ID must be a government-issued document that clearly shows the following:
- Your full name
- Your date of birth
- A clear photograph of you
You can often still use an ID that is recently expired, provided the photo is still a clear likeness of you and all your personal details (name and date of birth) are correct.
Examples of Accepted ID
The following forms of photo ID can be accepted as long as they meet the conditions listed above:
- Driving licence issued by the UK, any of the Channel Islands, the Isle of Man, or an EEA state (including a provisional driving licence)
- Passport issued by the UK, any of the Channel Islands, the Isle of Man, a British Overseas Territory, an EEA state, or a Commonwealth country
- National identity card issued by an EEA state
- Biometric immigration document or Biometric Residence Permit (BRP)
- Identity card bearing the Proof of Age Standards Scheme hologram (a PASS card)
- Ministry of Defence Form 90 (Defence Identity Card)
